One A4 page for smartphones and tablets: customer, device with IMEI, fault list to tick, unlock code or pattern, condition and accessories, data backup, price and deposit, notes and signature. At the bottom there's a tear-off collection slip for the customer.
You can print the form in your browser or save it as a PDF. You may use it for your business, adapt it and print it with your stamp or logo.
These are the fields on the phone repair order
| Field | What goes in |
|---|---|
| Customer | Name, phone, email and the address if needed – for queries and the message that the device is ready. |
| Device | Make, model and colour. The colour helps when there are several identical models in the workshop. |
| IMEI or serial number | Identifies the device clearly. Tablets without mobile data have no IMEI; there the serial number counts. |
| Handed in | SIM card, memory card, case, charging cable: whatever was handed in is ticked and goes back with the device. |
| Condition and existing damage | Scratches, cracks or a bent frame, noted before the repair. That way you can show at collection what was already there. |
| Fault details | Screen faulty, not charging, network problems, battery faulty, speaker, water damage, back cover broken, microphone faulty, software fault, won't start, drop damage – to tick. Anything else goes under “Other faults”. |
| Unlock code or pattern | Only if it's needed for the repair or the function test. For a pattern there's a grid with nine dots. |
| Backup | Done by the customer, not possible or not wanted – to tick. |
| Cost and date | Price or estimate, deposit, expected ready date and the approval: up to what amount you may repair without calling back. |
| Notes and signature | The customer confirms the order and notes. “Received by” shows who in the team took in the job. |
| At collection | Repair carried out, final amount, date and signature on return. |
| Collection slip | The tear-off section for the customer – more on this below. |
Together with the condition and accessories, the ticket also serves as a handover record: what was recorded at intake, you check again on return. For computers, electronics, bikes or watches, the general repair order template is a better fit.
The unlock code: only if needed – and never on the collection slip
Only ask for the code if you really need it for the repair or the function test afterwards. On the form it goes in the top part, which stays in the shop, not on the collection slip that the customer pockets and might lose. If the customer doesn't want to give the code, tick “no code given”. If they did give it to you, you can recommend they change it after collection.
In Taskly the code goes in its own “Password / code” field. In the link the customer gets by email and on the signing page, it only appears as “••••”. On the job itself and on the printout from the app, it stays readable for your team.
Data backup before handing in
Data can be lost during a repair, for example if a device has to be reset. So ask the customer to make a backup before handing it in – via their phone's cloud or on a computer – and tick on the ticket whether this has been done. If the device no longer starts, a backup often isn't possible; that belongs on the ticket too, so there are no misunderstandings at collection.
The liability note on data loss
Directly above the signature line there's a note that the customer confirms with their signature:
Please back up your data before handing in your device – data can be lost during a repair. As far as legally permitted, we accept no liability for data loss. Please remove your SIM and memory card if possible. We will hand over the repaired device once it has been paid for in full. We keep uncollected devices for ____ weeks after notifying you that they're ready. With my signature I place the order for the work listed above and confirm that I have read the notes.
This is a suggested wording and not legal advice. How far a pre-printed exclusion of liability holds depends on the individual case; liability usually can't be excluded entirely in pre-printed terms – hence the addition “as far as legally permitted”. Have the text checked for your shop, for example through your chamber or a lawyer. Day to day, what matters most is that the customer backs up beforehand and the ticket shows they were told to.
The tear-off collection slip
At the bottom of the form there's a collection slip. The customer takes it with them; you keep the top part with their signature. Only the job number, date received, device, expected date, price and deposit and your stamp with phone number belong on the collection slip. Enter the same job number at the top and bottom, and you'll find the device straight away at collection.
Digitally, in Taskly the link by email takes on this role: the customer sees the job number, device and current status – received, in repair, done, collected – and, as soon as the device is ready, the amount still to pay at collection.
Estimate for phone insurance: what goes in
If the phone is insured, the insurer often asks for an estimate from the workshop. Exactly which details it needs is in its terms or on the claim form – if in doubt, the customer asks there. Usual details are:
- Your business's name, address and phone, date, stamp and signature
- Name of the customer or policyholder
- Make, model and IMEI or serial number
- the damage as you found it
- Cause and date of the damage as stated by the customer – marked as such
- the costs, split into spare parts and labour, with VAT (without, if you're a small business exempt from VAT)
- whether the repair is worthwhile or the device is an economic write-off
Only write down what you've checked yourself. If you want to charge for the estimate, agree this explicitly with the customer beforehand: under § 632 para. 3 BGB, an estimate is not to be paid for if in doubt. This isn't legal advice either. On the form, the customer ticks whether they want an estimate before the repair or for the insurance.
Fill in the phone repair ticket digitally
Taskly is a digital work order for workshops and repairs. The phone repair ticket is available there as the ready-made “Phone & tablet” template:
- Record the device and fault. Type in the device, tap the faults – the same list as on the form. You can photograph the IMEI sticker or rating plate; Taskly reads the model, IMEI and serial number, and you check the values. The photo isn't stored for this.
- Code and existing damage. The unlock code goes in the “Password / code” field; you record scratches and cracks with photos at intake.
- Signature. The customer signs with a finger – on the tablet or via QR code on their own phone. If they gave an email address, they get the ticket as a link by email.
- Mark it done. One tap on “Ready” and the customer is notified by email. SMS is available on request for enabled businesses.
How signing on the tablet or the customer's phone works, and what Taskly stores as proof, is described under customer signature on a tablet or the customer's phone.
The template's note is pre-filled and can be changed freely. The phone template doesn't have a data backup field out of the box; you add it in the template builder, for example as a selection list. How the whole repair intake in the shop works with Taskly is described under software for phone repair. Taskly costs €19 a month, try it free for 30 days – pricing and limits · create an account.
What Taskly is not
Taskly replaces the paper repair ticket, not your till system: Taskly doesn't include a till, stock management with inventory, buying used devices under the margin scheme or a separate form for estimates. Taskly needs an internet connection to save – if the network at the counter often drops, the paper form is the more reliable choice.
Phone repair order FAQ
Is the template free, and may I adapt it?
Yes. You open, print or save the repair ticket, including the tear-off collection slip, as a PDF without signing up. You may use it for your shop, adapt it – for example with your own fault details – and print it with your stamp or logo.
Does the customer have to give me their unlock code?
No, that's the customer's decision. Without the code, though, a device often can't be fully tested after the repair. Then it's best to test it together at collection – and the ticket shows that no code was given.
Where do I find the IMEI?
On most phones, entering *#06# in the phone app shows the IMEI. It's also in the settings under the device information and often on the packaging. Tablets without mobile data have no IMEI – then enter the serial number.
What is the collection slip for?
The customer pockets it and brings it when collecting. With the job number you find the right device, even if several identical models are waiting. The unlock code and address are deliberately not on it – in case the slip gets lost.
What goes into an estimate for phone insurance?
Usually the business, customer, model with IMEI or serial number, the damage found, the cause of the damage according to the customer, the costs split into spare parts and labour, and your assessment of whether the repair is worthwhile. Exactly what's required is in the terms or on the claim form of the insurer concerned.
Is my shop liable if data is lost during the repair?
That depends on the individual case. The note on the ticket and the ticked data backup show that the customer was informed. However, a pre-printed exclusion of liability usually doesn't apply without limits. Have the note checked for your shop – this information is no substitute for legal advice.
Is the phone repair ticket also available digitally?
Yes. In Taskly the template is called “Phone & tablet”: the same fault list, a separate field for the code, photos at intake and the signature on the tablet or the customer's phone. Taskly costs €19 a month, try it free for 30 days – details under pricing.
